Cardio Oncologic Complexity in Gynecological Malignancies: Integrating Nuclear Radiation Therapy, Chemotherapy, and Radical Surgery in Pacemaker Dependents Patients

Abstract

Background: The management of gynecological malignancies has become increasingly complex in patients who are pacemaker-dependent due to the potential cardiovascular risks associated with chemotherapy, radiation therapy, and radical surgical procedures. These treatment modalities, while essential for cancer control, may adversely affect cardiac function or interfere with the performance of cardiac implantable electronic devices, requiring careful multidisciplinary planning.

Objective: This review aims to summarize the current evidence on the multidisciplinary management of pacemaker-dependent patients with gynecological malignancies, with a particular focus on the integration of nuclear radiation therapy, chemotherapy, and radical surgery. It also highlights the challenges, safety considerations, and recent advances in cardio-oncology that support optimal treatment outcomes.

Methods: A comprehensive review of the published literature was performed using peer-reviewed articles, clinical practice guidelines, systematic reviews, and observational studies focusing on gynecological cancers, cardio-oncology, pacemaker management, radiation oncology, chemotherapy-induced cardiotoxicity, and perioperative care. Relevant evidence was critically analyzed to provide an updated overview of current clinical practice.

Results: Available evidence suggests that successful treatment of gynecological malignancies in pacemaker-dependent patients requires individualized risk assessment, careful radiation planning, continuous cardiac monitoring, appropriate pacemaker management, and close collaboration among gynecologic oncologists, cardiologists, radiation oncologists, medical oncologists, anesthesiologists, and electrophysiologists. Modern radiotherapy techniques, advancements in cardiac device technology, and standardized monitoring protocols have significantly reduced treatment-related complications while improving patient safety and clinical outcomes.

Conclusion: The integration of nuclear radiation therapy, chemotherapy, and radical surgery in pacemaker-dependent patients presents unique clinical challenges that demand a multidisciplinary cardio-oncology approach. Early cardiovascular evaluation, personalized treatment planning, and adherence to evidence-based guidelines are essential to minimize complications and optimize both oncological and cardiac outcomes. Further prospective studies and standardized clinical recommendations are needed to strengthen evidence-based management strategies for this high-risk patient population
