Abstract

Mandibular edentulism significantly affects mastication, speech, and quality of life, particularly among elderly individuals. Implant-supported overdentures have emerged as a predictable and effective treatment modality to overcome the limitations of conventional complete dentures. While the placement of two implants in the interforaminal region is widely accepted as the standard of care, the selection of an appropriate attachment system remains a subject of ongoing debate. This narrative review aims to synthesize current evidence from PubMed-indexed systematic reviews and meta-analyses to evaluate various attachment systems, including stud, bar, magnetic, and telescopic attachments, in terms of implant survival, prosthodontic maintenance, retention, peri-implant tissue response, and patient satisfaction. The evidence suggests that although implant survival rates are comparable across attachment systems, significant differences exist in maintenance requirements and patient-centered outcomes. Locator attachments appear to offer an optimal balance between retention, resilience, and maintenance, making them a preferred choice in many clinical situations.
